Make float placement smarter

• keep the float close to where it is first referenced (place the float when it is referenced)
• float-only pages when there are many floats

Pagination could be improved. While the attached example seems technically correct and is probably not an "orphan", it would be nice if 1.1.6 was deferred and placed on the next page. I know about the manual page_break option.

@rweickelt These are indeed on the to-do list, but covered by #86.

Firgures and tables are examples of floats, which I have not defined here 🙂. These ideally would 'float' to the top/bottom of the page instead of appearing exactly where they are defined in the text.
