[CNCF Graduation] Document Complete Maintainers List in Uniform Format (Affiliation, Domain, GitHub)

• TEAMS.md provides TOC members, Team Leads, Maintainers, and Component Maintainers with GitHub handles and team assignments.
• GOVERNANCE.md#technical-oversight-committee describes TOC responsibilities and links to TEAMS.

Gap Identified

• Affiliation is not presented in a complete, uniform format for all listed maintainer roles (explicit for TOC, but inconsistent for Team Leads/Maintainers/Component Maintainers).

This criterion asks for complete maintainer listing including affiliation in a proper format. Current coverage is incomplete/inconsistent outside the TOC table in TEAMS.md.

Please update [TEAMS.md] or add a new MAINTAINERS.md to include complete maintainer info for all roles in a tabular, standardized format, similar to kserve/community/MAINTAINERS.md.

Ensure all current maintainers, Team Leads, Component Maintainers, and TOC members are listed with affiliation in a single, comprehensive resource (or clearly cross-linked tables).
