Assignments: Students can upload documents as comment to their own upload

When a teacher comments an assignment, the student can answer that comment and can attach a document. This leads to confusion as teachers consider the assignment was sent and the date prevented any additional document to be sent, and the students believe that they can send a second version of their assignment (and the teacher is not made aware that there is a document attached).

In the comments section for a submitted assignment, students must not have the possibility to upload files (in any way - not through CKEditor nor through a "Browse" button).
