Full file contents in the system prompt should be properly wrapped

Files defined in "options.context_paths" setting are put inside the system prompt with full contents but without any encapsulation; only under a markdown header. That may mislead AI as the files itself may also have markdown headers too. So maybe wrap those contents inside some XML tags per each file?
