feat: Open relevant Discord links internally

Problem

Sometimes, there are links posted in Discord channels that lead to a specific message or a thread, which when opened in concord, open the link in the browser

Proposed solution

The discord.com link is detected and the relevant message or thread or channel is opened.

Alternatives considered

Using the discord client.

Additional context

Messages or threads in other servers should open an error dialog saying that the user is not in the server to avoid implementing 'peeking' in other servers.
