CKEditor 45 support

I've converted the plugin to TypeScript and added support for CKEditor 45 here: https://github.com/TriliumNext/Notes/tree/develop/packages/ckeditor5-mermaid

Note that it also contains a change in which the Mermaid instance is not bundled, allowing for lazy-loading or deduplication of the library with the client app.
