Bug: App sometimes shows “connected” even after the wallet is disconnected

Describe the bug

In some cases, after a wallet disconnect happens, the app still thinks the wallet is connected. This makes the UI feel out of sync with what’s actually going on.

Steps

1. Connect using the Coinbase Wallet SDK.
2. Trigger a disconnect (for example by ending the session or interrupting the flow).
3. Return to the app and check the connection UI.
4. Notice it can still show “connected” even though the wallet is disconnected.

Expected behavior

When the wallet disconnects, the SDK should update state right away so the app can show the correct “disconnected” UI.
