Headings generate the same IDs

The auto generated headings (i.e. features, Bugfixes, etc) produce the same IDs in the final output (each H1, H2, H3, etc)., This could be corrected by including the version number along side the heading. (heading IDs are auto-generated by lower-casing the heading title, replacing non alpha-numerics with -. i.e. not appending the version number makes all the Features heading have the same ID assigned to them)

Also, headings should be generated at the appropriate semantic levels (i.e. only one #, and not skipping intermediate levels):

Using <a name="..."> anchor tags is no longer needed in HTML5 (as the heading ID provides the same functionality).

I think this change should be done in https://github.com/conventional-changelog/conventional-changelog/tree/master/packages/conventional-changelog-writer

You might need to tweak the the handlebars js template too. But let's fix the writer first :)

Removing <a name="..."> should be a breaking change. since tools like this relying on it

maybe <a name="..."> should use a generic name value rather than relating it to the version number (HTM DOM methodsL don't like ID's or link targets starting with a number (i.e. document.getElementById('0.1.2') will fail)

I'll take a look at teh source and templates to see how they can be tweaked.

All I really know right now is that the duplicated auto ID's assigned to the headings (which is based on the heading title, via markdoen conversion to HTML) makes it impossible to link to the specific bug fix (or features) sub headings.
