When accepting a held message... it should be very easy, perhaps even "the default" to unmoderate the user by moving them from "List Default" (which happens to mean moderated, although is configurable) to "Default Processing".

For example, add a checkbox, when accepting a moderated/held message, which says "And also unmoderate the member", and this is selected by default, so it happens automatically.

An open question is if this functionality is hardcoded to toggle between "List Default" -> "Default Processing", or if it's more configurable, someplace in the settings, as "Moderation Group A" -> "Moderation Group B", and then if the whole thing is enabled or not. It's reasonable that such functionality is opt-out or opt-in.
