Issue Description

Given: The design work for VHA payment history has been researched, updated, and finalized. The engineering team have written tickets to get started with implementation.
When: We are in the final stages towards implementing the improved VHA payment history experience
Then: We need a final layer of stakeholder feedback and we plan to gather written feedback through a survey.

Tasks

• Create survey based on survey questions provided by @charlottecesana from [VHA Payment History] SME/Stakeholders written feedback for VHA payment history (survey) #102621
• Update previous SME survey with new questions (https://tally.so/r/3jLjJ1)
• Capture screenshots for survey questions

Design and Front End sync on implementation of design
