Spike: 10-7959a multiple claims

As a 10-7959a applicant I want to include more than 1 claim on the form so that I don't have to keep filling out a separate form for each claim.

• generate a copy of the PDF per claim
• allow user to enter more than 1 claim (following loop pattern in the figma)
• user can submit different claim types: medical claim or prescription claim

Acceptance criteria:

• Given the applicant has multiple claims to submit
• When they fill out the form with all their claims
• Then the form should generate separate PDFs for each claim

Frontend portion of this covered in this ticket. PDF generation is a backend task, but with the frontend in place BE will be able to move on this.
