Design: 10-7959c business design changes needed to OHI cert Medicare/Health Insurance

Update the 10-7959c OHI cert in Figma to include the following fields to accommodate the business needs to process the certification properly:

Part D questions :

• Add the termination date (non-required field). Remove the carrier name.
• Add the Part D questions to the end of the Advantage Plan flow if they don't select that they have pharmacy benefits?
• We need to look at the wording to clarify this from included drug benefits in an Advantage plan

Part A and B questions:

• Remove the carrier name for A and B

Health insurance:

• Remove the Prescription Discounts from the insurance types list.
