Procure the external resources and approvals needed to add Datadog RUM to form 21-4138. This ticket unblocks the dev work in #6111.

Tasks

• Request a new Datadog RUM application from the VA Datadog admin team on ddog-gov.com
  • Service name: simple-forms-21-4138
  • Capture the applicationId and clientToken
• Register feature toggle form214138BrowserMonitoringEnabled in Flipper, default off
• Schedule OCTO privacy review for the session replay configuration
  • Form collects SSN, addresses, DOB, free-text statement narratives, and file uploads
  • Baseline masking approach: defaultPrivacyLevel: 'mask-user-input' + data-dd-privacy attributes + beforeSend click anonymization (matches CHAMPVA)

Acceptance criteria

• Datadog applicationId and clientToken in hand and shared with the dev team
• form214138BrowserMonitoringEnabled registered in Flipper (off)
• OCTO privacy review scheduled or complete, with any feedback documented
