Limit how many issues the region rule reports

Pages without a landmarks can end up having hundreds of issues reported by the region rule. This can be overwhelming, and isn't a good representation of the problem. A site that's missing a header, main, and footer landmark has 3 issues, not 1000.

I'm not sure what algorithm to suggest here to address this. Maybe work out common ancestors?
