Document how the same index allocator (and backing storage) can be shared across multiple issuer instances

This includes enabling safe sharing of index management across issuer instances that might be configured to output differently formatted VCs (DI proofs vs. some envelope format). Configuration can get very complex though, especially when considering systems that might use more than one status instance.
