Description
When setting up invite form fields and later on initiating an invite, the fields show up without their description or title - this makes it very confusing for the person inviting to fill-out.
In a document where multiple fields can be part of the invite form fields section (e.g. start date, end date, amount 1, amount 2 etc.) the user is unable to know which fields he/she are filling out without the names of those fields showing up. Yes, you can set default values for text fields as pseudo-labels but you cannot do that for non-text fields.
Field names (titles) should show as part of the submission screen to the left of each respective field as in the second screenshot below.
