Bug: codeblocks with ´linenums´ option that are too long to fit a page break the PDF generation

Hello !

I'm having an issues when using large codeblocks in my documentation. It only appear when the linenums option is used and only affect codeblocks that are too long to fit the height of the PDF page.

It will completly remove all content that should appear after the codeblock. It will also result with the same ToC issue explained in #35.

I'm trying to use Vivliostyle. (JavaScript-based, using Chromium)

The code blocks are cut off at page breaks. and page numbers are fine.

I'm going to start investigating the Vivliostyle code.
