Adding pg_duckdb to Azure Managed Postgres

I've seen this issue mentioned a few times in #52

it would be great to have this extension added to azure.extensions
in https://learn.microsoft.com/en-gb/azure/postgresql/extensions/how-to-allow-extensions?tabs=allow-extensions-portal#extension-versions Though i am not sure how that would need to happen
