[Issue]: Integrations Server missing from Stack monitoring docs

What happened?

The Visualizing monitoring data section is missing pages for the "Integration/APM server" stack monitoring panel(s).

In addition, "Integration Server" can be confusing as it used to be called APM server and the data still represents the APM server in context of the integration server.
