Tools: Tool version scan can be duplicated

**Describe the bug**
`tool_version_scan` SQL table doesn't have constraint on unique pair (tool_id, version).
This could lead to existence of two version scans at the same time, which will also lead to having duplicates of tools in some api methods, like `/dockerRegistry/loadTree`
