Everything issues with Signup page

Here's everything issues I'm facing with daftar.php that I couldn't solve it.

1. Cannot submit data.
2. Failed check whether email already in database or not.
3. Failed to check whether "Retype Password" field are same with "Password" field or not.
4. Issue with IDAnggota.
