Credit Caleb Dean at the top-level framing layer

Strategy 8 already credits Caleb Dean, Runify, and the Superwall Podcast
inside the strategy body itself, but two top-level framing locations
still implied Greg Isenberg was the sole source of the entire playbook:

- playbook.md preamble's single-source line → now lists both Greg
  Isenberg (Strategies 1-7) and Caleb Dean (Strategy 8) with a link to
  the Superwall episode.
- README.md strategy list → Strategy 8 now carries an inline credit
  parenthetical so a reader skimming the README sees the provenance
  without drilling into playbook.md.
