Location scoped data file attributes render poorly on location editing page #127

Description
What steps will reproduce the problem?
1. Add a Location-scoped attribute of type Data File to a survey.
2. Edit a location for that survey.
What is the expected output? What do you see instead?
Note that the rendering of the input field is poor as illustrated in the
attached image.
