Code review persona

Have you considered adding an explicit code review persona?
Maybe this would be a stepping stone between the polecats and the refinery. All MR's would have to be reviewed. You could optionally use a different or higher-end model on it to provide a 'second set of eyes' (i.e., OpenAI reviewing Claude). Failed reviews would just become further work beads.
Refinery could have some 'rules', ether default or changeable as to the level of defect that could be let through, and the circumstances under which it was escalated.
