MacOS global hotkey stopped working in 15.4

My global hotkey for opening the quick terminal was working fine until I upgraded to MacOS 15.4 yesterday. Now it only works if Ghostty is active. I've tried resetting the accessibility permissions and rebooting without success.

There are a couple of closed issues and discussions of the similar issue with no resolution: #2390, #5080.

I'm unable to reproduce this. I'm moving to Q&A, since previously the fix has been to clear your AX permissions for the app and start from scratch. There's a CLI command you can use to do that but don't have that off hand. If we're able to get a reproduction or more reports of this then we can move back to issue triage.

I think I had the same issue even though the Settings > Privacy & Security > Acessibility permission was given to Ghostty as said in the docs. But I got it to work again after enabling the Settings > Privacy & Security > Input Monitoring for Ghostty (like in #2390's screenshot). You can try enabling it if you haven't already, and hopefully that'll work.

Maybe this additional permission should be stated in the docs for troubleshooting if the Accessibility permission alone doesn't work?
