Hi gang! I have recently come across1 the Markdown ADR standard: https://adr.github.io/madr/
(As recorded2 in ADR000, the existing template used in this repo is based on Michael Nygard's format.)
I immediately thought of the forms team, and wonder if this might be a useful tweak for your process into what might be a more well-understood (and certainly well-documented) format.
There is even a cli tool for MADR... https://adr.github.io/adr-tooling/
