Clean up README deployment instructions

There's some inconsistent/duplicate steps in the README that render the deployment path for a fresh instance less than clear. For instance, I think the `prep_new_prod` task supplants a lot of what's in the numbered instruction, but those have not been removed, causing (me!) confusion.
