Description

A capture template that adds provenance to claude.ai-to-Claude-Code handoffs: model recollections and suggestions are tagged ([MEMORY]/[PROPOSED]) and never presented at the user's confidence, unstated scope choices are listed for review instead of labeled "locked", and a verbatim appendix preserves the raw input. Includes a case study diffing six real handoffs against exact transcripts, in which every downstream correction traced to unmarked model additions, not omissions.
