Context: Creating new items directly from the quick access palette mimics a flagship 1Password feature, though UI complexity is high.

Acceptance Criteria:

• Implement a "New Login" form page.
• Utilize Adaptive Cards (or similar UI constructs) to gather fields.
• Save the new item via bw create item.
