Docs: Add guidelines to CONTRIB on filing Issues #449

Description
We'd like Issues to be posted with as much information as possible, first time, so we hopefully don't have to re-visit them and ask for more input etc. (or at least reduce that).
To that end, we should work out some 'Issue posting guidelines' and add them to our CONTRIBUTING file.
