Restore RA and Data Provider Harvest Sources from Registry to CKAN

We discussed during today's meeting: we need to get the full inventory of the harvest sources configured in the former Harvest Registry in order to migrate them over to CKAN to ensure we don't lose any data.

We don't need to do the CKAN migration currently, as we're still solidifying the harvest-related issues in CKAN, however,

@benjwadams can you post a database dump of whatever the Harvest Registry table was used to store harvest sources (organizations/providers and harvest source URLs) directly here in this issue in CSV format (or markdown table format if practical)?

@benjwadams Raising this issue again since at this time the harvests are running better per ioos/ckanext-ioos-theme#238, can you post a database dump/email of the former Harvest Registry harvest sources so we can officially migrate them to CKAN?
