Add new CKAN user failure

What went wrong?

I go to an CKAN Organization page to create a new 'member' of an existing Org, fill in user details:

and upon clicking the 'Add Member' button, I get an 'Internal server error' message on the resulting page.

It seems user creation capability is down. Could this be a result of the work done to disable the option for public users to create their own accounts just by browsing to the Catalog (disabled over ~1 yr ago, not able to fiind the relevant issue if it existing for that change)?

We need to have the ability as a CKAN administrator to create new user accounts to manage harvests.

Code to Reproduce

Go to Organizations > IOOS page.  Click on Manage > Members.  Click on Add Member to get to the above screenshot page.  Enter user info, click Create Member.

I also asked on the CKAN [gitter.im channel](Gitter | ckan/chat) about adding a new user. While logged in as an admin, you can go to Organizations > REGIONAL_ASSOCIATION_NAME and then click the "Manage" icon.
After that, click the "Members" tab and there is a form field to fill out to invite a particular email address to join a given RA via an email invite. I checked the config and there are email credentials present, but I have not tested nor sent an email through CKAN recently. Please see if these work to add Laura to the appropriate organization.

I did these steps originally (see screenshot/details above), but tested again today and received the same 'Internal Server Error' message that I did the first time.

Here's a screenshot, although it doesn't add much beyond the above.

Perhaps the email service is down as you mentioned? Hopefully the system logs will give you some insight. Please investigate.
