expose lag delta so it can be more clear when providers are under-provisioned

We can see a current lag seen by the indexer. with multiple data points (the most recent lag, and the lag at previous fetch) we can know if the speed of fetch is slower than the speed of index chain growth.

this may need to happen / be worked around the double cursor ingest
