State insurance department and consumer-affairs portals

One-page lookup of the right complaint portal per state. Used by the drafter to fill complaint_state_doi.md placeholders (agency name, mailing address, online portal URL, attorney general fallback). Order the kit picks:

  1. If the bad actor is a licensed insurer → state DOI (insurance department).
  2. If the bad actor is a provider, hospital, or third-party debt collector → state Attorney General's Division of Consumer Affairs (sometimes called Consumer Protection).
  3. If the bad actor is a non-profit hospital not meeting § 501(r) → IRS Form 13909 (federal), in parallel.
  4. If the bad actor is a hospital out of compliance with 45 CFR Part 180 → CMS HPT complaint (federal), in parallel. See templates/complaint_cms_hpt.md.

Entries are listed alphabetically by USPS code. Each entry shows the DOI portal first, the AG consumer-affairs portal second. URLs are the public submission pages. Update on a yearly review; portals are rarely renamed but office addresses do shift.

How the drafter uses this file

The drafter resolves the patient's home state (from HEALTHBILLS_PATIENT_STATE) and the biller's state (from BILLER_STATE_OVERRIDES if set, else assume patient's state). It picks DOI or AG based on the bad-actor classification:

  • Insurer (the EOB-issuing entity) → DOI of the patient's state.
  • Hospital, physician group, or other provider → AG Consumer Affairs of the state where services were rendered.
  • Third-party debt collector → AG of the patient's state AND CFPB at the federal level.

States not listed above (AK, DE, HI, ID, ME, MT, ND, NH, RI, SD, VT, WV, WY, plus DC and territories) are deliberately omitted from the initial public release of this file. They follow the same DOI / AG pattern; the drafter falls back to references/laws_state_template.md and asks the patient to confirm the agency name. Pull requests to fill these gaps are welcome.
