'Submit CDN' Dialog remembers the last submitted thing #75

the 'Submit CDN' dialog stays populated even after the dialog is closed, the next time it opens it's doesn't start blank, but has the same information used last time. So it's often necessary to manually delete the fields. It's not a big issue, though it makes it more likley for user to make mistakes.
