Summary

The identicon (avatar/icon) is missing for the additional mount folder entries in the ModelService creation form.

Steps to Reproduce

1. Open the ModelService creation page
2. Add additional mount folders
3. Observe that the identicon is missing/not rendered for the additional mount entries

Expected Behavior

Each additional mount folder entry should display an identicon, consistent with other similar UI elements.

Actual Behavior

The identicon is absent for the additional mount folder entries.
