Update Documentation for v4 including a new Migration Guide

• minimum preparatory work for a new docs tree. Example: Duplicate docs tree for next major laminas-paginator#75
• Initialise an empty migration guide in the v4 docs tree so that BC breaks can be documented as they are worked on
• General improvements and adjustment to docs as required by code changes
