[Docs] Clean up `CHANGELOG.md`

Current State

The CHANGELOG.md have multiple commits of the same commits from the releases.

Once we move to single package repo, but still keeping in mind of any other packages that we may need as separate use case, the CHANGELOG.md needs to be updated.

@Nabanita29 Thanks for reaching out. This issue may not be needed right now, as we moved to single package, and the deprecated CHANGELOGs have been moved to its appropriate directory for now. Since we're using GitHub Release to initiate the release, we will be relying on the release notes for this.
