If I provide my own instruction file using the --instructions-file flag, does it override the default instruction file, or is the custom instruction file used in addition to the default one?

Yes, it will override the default instructions.
Please note that your instructions file should follow the same structure (headers/format) as the default one:

Yes, --instructions-file fully overrides the default instruction file. Your custom file replaces the bundled instructions entirely — it is not appended to them.

If you want to build on the defaults, you can copy the bundled file from whisperjav/translate/defaults/standard.txt (or pornify.txt) and modify it as your starting point.
