Sometimes the grub-reboot entry doesn't get picked until an update-grub is run

A behaviour I've noticed inconsistently.

Sometimes I'll pick Windows from the entries, but after the reboot, Grub still has Linux selected.

I have to run sudo update-grub and then when I pick Windows, it works and Windows is selected on next boot.

Unknown if it's related, I do have GRUB_SAVEDEFAULT=true and DEFAULT=saved

But no idea what the problem is right now.
