Connecting through Object explorer results in old palette prompts for creating a firewall rule

Repro:

1. Create a connection to an Azure server
2. Go to the Azure portal and remove the firewall rule that allows you connect
3. Disconnect and reconnect

Expected:

• Prompted with new UX to add firewall rule

Reality:

• Command palette prompts, no integration with vscode accounts.
