BUG - Chat history disappears when you refresh the page / navigate to a previous chat and then send a new message

I think this is a critical bug since it effects chat history which is a fundamental part of this repository and any AI chat.

Steps to re-produce:

1. Send some messages in a chat and then refresh the page or navigate to an existing chat

2. Send a new message

• (BUG) The previous messages will disappear, only the new messages show
• Refresh the page and the old messages re appear and the new messages appear

For example:

1. Navigate to this chat: https://demo.ai-sdk-tools.dev/pMqYM5CKjYUdLTJD

2. Send a new message "tell me a joke"

• (BUG) The chat history disappears
