feat: freeze Phase 4, mark 4-3 as trigger-gated

Phase 4 is frozen after completing evidence-graded theory drafting (4-1) and
theory draft skeleton (4-2). Phase 4-3 is trigger-gated: 0/8 triggers met,
will only reopen on corpus growth, intervention tag accumulation, failure/
near-failure density, or metadata coverage crossing predefined thresholds.

Update all research status markers consistently:
- Phase 3D: complete
- Phase 3E: complete
- Phase 4: frozen (not active; 4-3 trigger-gated)
- Phase 5: not open
