The Checklist is missing from a single form (User Story) on a single project. How do we restore it without affecting other projects and any associated lists

[Resolved] We suspect someone may have deleted it from the user form. We can still create checklist items, but the Extension group is missing from the User Story in the custom process template, so it is not displaying the list for the user story WIT. Can this be restored by uninstalling and then reinstalling the extension without impacting any other projects?
