[CCEI / GAPS] Make 'Reason' field mandatory for 'Not functioning' status

UNICEF have requested that users must capture details about why a CCE is not functioning.

Describe the solution you'd like 🎁

The 'Reason' field should be mandatory when selecting the 'Not functioning' CCE status.

In addition a new Reason Other should be available. If the user selects Other then the 'Observations' field must be mandatory.
