How to pair MD with Jupyter Notebook when contents are different

Say I have MD file which is describing the report, which will have different content to Jupyter Notebook, so how do we pair them so that results in Jupyter Notebook is reflected in MD, but at the same time MD describes in words about the project which will not have the Python Code inside?
