Reorganize OA4MP QDL distro PDFs

The standard QDL distro for OA4MP comes with a ton of PDFs -- over 2 dozen. The issue is now that there should be probably a directory structure and table of contents (TOC) for quick reference to navigate them. Also as part of that should be some tips on reading them from the command line (like pdftotext working with less).

Also, every PDF should have a reference on some page of the website. Make sure they references are up to date and make sense.
