New issue forms!

We've opted to participate in testing GitHub's new issue forms. These are structured web forms which replace the old Markdown bug report and feature request templates. They should provide for a smoother experience when opening new issues.

Nice as they are, keep in mind this feature of GitHub is still being tested. Please comment here if you run into any problems submitting new issues so that I can look into it. Thanks!
