External links - how do we distinguish from internal links? should we?

At the minute we don't treat links that go to external sites any differently to internal links on the site. Is this something we should look to do?

Sometime's see external links indicate with an icon or text in the link.

What would constitute an external link? Would a link from NICE.org to CKS be external or internal?
