Looking into this a bit. We should check whether these FLAIRs are high enough resolution:

flair = None
if flair_list and not t2w and max(nb.load(flair_list[0]).header.get_zooms()) < 1.2: 
    flair = flair_list[0]

Also posted to the linked Neurostars issue. It's possible we just forgot about this constraint and that's the root cause.
